Recently had email hacked. Emails sent to any and all addresses in my contacts. How do I know...

New hard drive installed. Was not receiving emails but could send. Have ATT server with yahoo mail but used Windows Live mail. Thought problem was solved via ATT tech support on the phone. Next day found out that could now receive but not send. Finally going thru numerous ATT sites found two with suggested changed in settings. However, each was different. Tried one and did not help. Tried second and I was able to send and receive. Next day had messages from friends questioning a strange email with links I had sent to them. Had not even been in email that day. Then successfully sent to everyone on my email contacts list for 3 days. Finally went through ATT and they helped me change passwords (site would not let me do that before). I'm pretty much computer challenged so I would like to know if this could have been key logging, phishing or just plain hacking (I don't know the differences) and if anything else should be done. (Have run Malware Bytes and then Microsoft Security Scanner.
