The option used to be there for everyone. But there must be some error in the permissions or something.

Ok, To settle everything, Custom titles is only a function of green members. If you are not a elite member then you don't have the option to get a custom title. As for the PM's, I am not sure what exactly is wrong, nobody has access to the confirmation because I have turned of that functionality all together.
